A Clinical Analysis fo Primary Malignant Tumors of Duodenum

Abstract
We analyzed retrospectively 49 patients with primary malignant tumors of duodenum diagnosed at the Seoul National University Hospital from January 1980 to April 1991.
@ES The following results were obtained.
@ES 1) The mean age of patients was 56.4 years. The peak incidence was 6th decade in male and 7th decade in female. The male to female ratio was 1.9 : 1.
2) Seventeen cases of duodenal cancer were located in the 1st portion, 18 in the 2nd portion, 8 in the 3rd portion, and 6 in the 4th portion. Suprapapillary cancer was found in 19 patients, peripapillary cancer in 17 patients, and infrapapillary
cancer
in 13 patients. Histologically, 46 patients were adenocarcinoma 2 leiomyosarcoma, and 1 lymphoma.
3) Main symptoms and signs were weight loss, abdominal discomfort or pain, and jaundice.
4) Gastroduodenoscopy, UGI series, and abdominal CT had important roles in the diagnosis of duodenal cancer.
5) We performed 12 radical operations(10 Whipple's operations, 1 pyrolus-preserving pancreaticoduodenectomy and 1 segmental resection of duodenum) and 17 palliative operations. Overali resectability was 41.4% and resectability of 2nd and 4th
portion
cancer was higher than that of 1st and 3rd portion cancer.
6) Operative morbidity and mortality were 21% and 7% respectively. The major postoperative complications were leakages at the anastomosis site in the curative resection group and acute renal failure in the palliative group.
7) In curative resection group, one, three, five year survival rate were 87%, 50%, 17% respectively. The survival rate of the curative resection group was better than that of the palliative group(p<0.05).
